not able to solve insert operation using bonita bpm ui to a mysql work_bench????

Still not able to insert data into a mysql workbench using bonita ui(7.2),I am trying this from last one week but still not able to solve....please please help me ,may be i am going some where wrong...so please if someone have example for this insert opertaion , please explain or post in community i will
take help ..........thank's in advance

HI, Sean McP AgainI want u help ,but with different problem .....

     problem is that I want to make report of database field using bonitasoft bpm and jasper connector but I am not able to do it...(I followed document also then also i did't got it) so,its my pleasure if u reply or give some idea's....please

thanks Sean McP , I have solved my problem, thankyou very much.....this happen just because of u...now i am able to insert data into database...
