Custom header for Living Applications

Please, someone can help me to find the custom V3/V4 for Living Applications header customizations? Some time ago I've found it but now I need it and I cannot find it back.

This one is the BEST answer!

You can import the Bonita User Application from the Studio Welcome page. It contains the editable UID layout page.
