Add document to an array

Hello there,

I want to create an array with the following data:

revision date, status and the revision document.

I have created in the business data model the object with the revision and status variables (I cannot add a document), therefore, when creating the form and pressing the button to add to the array, the new element does not have the option of "document "(that is, each new element of the array should have 3 fields: revised date, status and the document).



How can I do this?
