404 Error on loading of form and click on Submit Button

Hello, I'm having trouble with the submit form of a button. Nothing happens after the submit button is clicked and I get the following error on the browser console:

"Failed to load:
http://localhost:8080/bonita/portal/resource/process/Gestion%20de%20Cita...

resource: the server responded with a status of 404."

I expect it has something to do with the execution tab, given the failing URL. I have set the data model as variables on the contract for the pool, lane and specific action box, and I've assigned the same UI Designer form to the process instantiation for the pool, lane and specific action box as well. If any more information is needed to expedite the solution, I will gladly provide it.
