i dont know where i did wrong after i published, the create new request button works but when submitting to initiate new process, it does not submit in application, what could be the problem, any help would be greatly appreciated ?

Open the Page in Google Chrome , Press F12 , Then Click on the Button.
Navigate to the Network Tab in the console and check the Error (if any).
Usually an instantiation error comes with message contract violated .
Else check Error Logs .
