The widgets from the form builder of Bonita 5.9.1 cannot be extend.
What I would recommend is to consider a migration to Bonita 7 that with the UI Designer offer the capability to create and use your own widgets (or reuse one of the community).
If migration is not an option, you can create your custom forms with the technology of your choice and integrate them with Bonita.
