Run the process but no instantiation form

Hi all,

I'm follow the tutorial of the community version, I have been stuck at the step 3 of the "Run the process" :

3. On the instantiation form, click on Start. The process instance is started and you are redirected to the Bonita User Application task list

I don't see the instantiation form when I start the process pool. Could you show me how I  can do correctly ?
