upload CSV data to database

Hi,

I have a CSV and I want to upload its data to database. For this I created a document of CSV type, which is reading CSV when I browser the button, but I cannot insert its records to mysql database. I am using community version 6.4

Actually my client wants to provide a upload functionality on a form(through process) which browser for csv file and upload it to mysql, not using MySql Workbench.
