Connecting Application Pages via Link Widget

I have an Application Page that displays a list of objects. I've added a View/Edit link for each object (using the repeat container pattern) to allow viewing and editing of the details of the object using a different View/Edit Object Application Page that I've created.

On the Link Widget's properties, I've set the Type to Application page, but I can't find any documentation on (or examples of) how to set the three additional properties (Page Token, Application URL Token, URL Parameters). I assume Application URL Token is where to put the reference the View/Edit Object Application Page that I've created, but I don't know what it's URL is.

Any help would be appreciated.
